  • 1098 T Form

    You must have a U.S. issued Social Security Number (SSN) or Individual Taxpayer ID Number (ITIN) to be eligible for the 1098-T tax form.

    1098-T tax forms are available by January 31st each year. These forms are NOT mailed, and must be accessed online: login to myPortal > click on the Student tab > click on 1098-T Form Printing (located on the left-hand side).

     

    If your 1098-T form is blank, please enter your SSN or ITIN on myPortal.  

    *Social Security Number (or Individual Taxpayer Identification Number) is required for the 1098-T form. 

    • Login to myPortal > Student tab (top) > Student Account Center (left) > Update SSN. 
    • March 20th is the deadline to submit your SSN or ITIN in order to obtain your prior year’s 1098-T. 
    • If you submit your SSN or ITIN after this date, please provide your tax preparer with your student account history instead.

    NOTE: F-1 visa holders are not eligible for 1098-T tax forms.

    Please complete this Dependent I-20 Request Form to add a dependent. As soon as you complete the form, you will be in the queue for processing.

  • Change of Address

    Please complete this Change of Address Form to update your address and SEVIS record. As soon as you complete the form, you will be in the queue for processing.

  • Change of Location

    To request to change your location, please complete this Change of Location Request Form. This form does not guarantee your request will be approved. You will hear back from the graduate team with a decision on your change of location request approval or denial.

    To request to change your major, please complete this Change of Major Request Form. This form does not guarantee your request will be approved. You will hear back from the graduate team with a decision on your change of major request approval or denial.

  • Change of Name

    Please complete this Change of Program End Date form to request the change of your program end date.

  • Change of Visa Status

  • Extreme Circumstances Request

    Please complete this Extreme Circumstances Request Form if you have an extreme circumstance preventing you from attending your seated class. As soon as you complete the form, you will be in the queue for processing.

  • Find my Advisor and DSO

    To find your advisor and DSO, please go to this Find My Advisor and DSO Form page and enter your Trine student ID. 

  • Find my Class

    To find your seated class information, please go to this Find My Class Form page and enter your course code and section. 

    Please note, this will include seated class dates and times, instructor information, and class locations. 

  • Graduate Invitation Letter

    Please complete this Graduation Invitiation Form to request a letter for your visiting guest. Upon completion of this graduation invitation request form, you will receive an email with an attached invitation letter for your guest. You will need to complete this form for each guest you would like to invite.

  • Health Insurance

    It is required to have health insurance from Trine University. What is the coverage? 

    Per university policy, it is required to have health insurance through Trine University as an F-1 visa holder. Students are enrolled in health insurance automatically, one week after classes begin.

    Cost per semester is:         

    Fall: $490 (August - January coverage)
    Spring: $780 (January - August coverage)
    Summer: $365 (May - August coverage)

    To waive Trine's international health insurance, you must show proof of sponsored health insurance, which would be from your government, your employer, or your previous university (if the international health insurance is still valid). Waiver instructions will be given during the orientation webinar and he International Graduate Orientation modules. Due date for insurance waiver is the Wednesday before classes. Any waiver requests after the due date will not be approved.

    Note: This is only required for and only given to F-1 visa holders, Other visas are not eligible. We do not provide coverage for F-2 dependents. Students must contact UHCSR directly within 30 days of insurance registration to add dependents. 

  • Applying for SSN

    To obtain an Social Security Number (SSN), you must first have a job offer. Once you get a job offer, and it is approved by the Office of International Services (OIS), you will need to make an appointment to apply for one at your nearest Social Security Office. Most are requiring an appointment due to COVID. You will be required to provide the following:

    1. Passport
    2. Visa (does not apply to Canadians)
    3. I-94
    4. I-20 with Work Authorization
    5. 2 Proofs of Residency (we can supply one for you if you fill out the Proof of Residency Form)
    6. SS-5 Form (SSN application PDF form)
  • IT/Moodle Questions

    For questions about technology including computer issues, account login information, Moodle, or other information technology questions, please visit the Information Technology Services portal.

  • Post Completion OPT Request

    Please login to the Office of International Services Login Page to complete the Post Completion OPT Request.

    Once logged in: 

    1. Navigate to the menu labeled "Student Services" on the left
    2. Select the Post Completion OPT Application
    3. Enter your desired OPT start and end date

    As soon as you complete the form, you will receive an email confirmation and be in the queue for processing. 

  • Official Trine University Transcript Request

    The Office of the Registrar provides official transcripts for Trine/Tri-State University students and alumni. To request a transcript you will need to create an account through the Parchment website. The cost for this service is $10 per transcript.

  • Orientation Modules

    Orientation Module can be found in Moodle on your student dashboard. Trine University international graduate students are required to complete these modules by the end of their first term. 

    To complete these modules, please note you must work through each section by reading and answering all required questions before moving onto the next section.

  • Pay or View Bill Information

    Bills are always due the Friday before classes start.

    Failure to pay your bill by the due date will result in your classes being dropped and dismissal from Trine University.

    To view your bill:

    • Login to myPortal > click on the Student tab > Student Account Center > View or Pay My Bill.
    • This will take you to your Nelnet account, where you can view your bill 24/7 and pay your bill, all in one place.
       

    Payment Option #1: Pay your balance in full via one-time payment.

    To pay with U.S. funds:

    • In your Nelnet account, click on “Make a Payment” and enter all of the requested information.
    • American Express, Discover, MasterCard and Visa are accepted online with a 2.85% transaction fee.
    • eCheck (no additional cost) – ACH payment directly from your U.S. checking or savings account.
    • To pay with foreign currency:
      Go to PayMyTuition or Flywire and enter the requested information.
       

    Payment Option #2: Enroll in a monthly payment plan.

    • In your Nelnet account, click on “Set up a Payment Plan”.
      There is a $35 fee to enroll. NOTE: you must re-enroll each semester that you need a payment plan.
    • Payment Plan options: 6, 5, 4, or 3 month plans are available (depending on when you enroll).
    • Payments are processed automatically on the 1st or 15th of each month, depending on which date you select.
    • Once enrolled, your “Payment Plan Remaining Balance” will reflect on your bill.

  • Proof of Enrollment

    Please complete this Proof of Enrollment Form. You will receive your request letter/form within 24 hours to your email.

  • Proof of Residency Letter (DMV)

    Please complete this Proof of Residency Form. As soon as you complete the form, you will be in the queue for processing. 

  • Summer Break Request

    All students on an F-1 visa may leave the United States during summer break. No support letter is necessary.

    Please make sure to have a travel signature on the back of your I-20. The signature must be less than one year old upon your return to the US. If you need an I-20 with a new travel signature, please complete the I-20 Request Form.

  • I-20 Request

    Transfer students will receive their Trine I-20 two weeks after classes begin. Requests from transfer students will not be processed until then. Transfer students should not complete the form below.

    Please complete this I-20 Request Form. As soon as you complete the form, you will be in the queue for processing.

     

  • RCL Request

    Please complete this RCL Request form to request an RCL (Reduced Course Load). 

  • RFE Request

    Please complete this Request for Evidence (RFE) Form. As soon as you complete the form, you will be in the queue for processing.

  • SSN Support Letter

    Please complete this Social Security Letter (SSN) Request Form. As soon as you complete the form, you will be emailed your letter.

  • STEM Approved OPT I-20 Request (STEM EAD Required)

    Students can request an I-20 after receiving a STEM EAD card.  This allows us to verify their SEVIS record has been updated by USCIS.

  • STEM Extension or Employment Update Request

    Please complete the STEM Extension or Employment Update Request form if you are: 

    1. Applying for a STEM extension
    2. Updating your employer information

    As soon as you complete the form, you will be in the queue for processing.

  • Transfer Credit Request

    Please complete this Transfer Credit Request Form to request transfer credits from another university.  

    NOTE: This form should be used by current Trine University students wishing to request a transfer of graduate credit coursework from another school to be accepted at Trine University. 

    • Course transfers must be equivalent to Trine University courses. This is reviewed by the graduate programs team. Not all course requests are granted.
    • Course transfers are only accepted from US institutions.
    • Evaluations will be completed after your first successful semester at Trine University.
  • Withdrawing from Trine University

    Please complete this Withdraw Form if you are withdrawing from Trine University. This includes if you are transferring to another university, leaving the country, or changing visa status.

  • Should be Empty:
